How to Apply for the AFBS Accident on Set Program


Ontario Saskatchewan Nova Scotia Prince Edward Island Alberta Northwest Territories Nunavut Quebec Newfoundland & Labrador New Brunswick Manitoba British Columbia Yukon Territory

Workers' compensation is optional in the Province of Ontario for performers; however, stunt performers are excluded from this program. Accident on Set program covers ALL performers including stunt performers.

  1. Complete and submit the Accident on Set Application Form to Actra Fraternal Benefit Society

  2. As required, complete and submit the Workplace Safety & Insurance Board (WSIB) Employer by Application - Entertainment Industry Form. This form MUST be completed by all Ontario productions that are insuring their production crew through WSIB. When using Entertainment partners (EP) payroll service submit this form directly to them. When EP is not handling payroll, send this form to AFBS, who will forward it to WSIB on behalf of the production.
